This is an assessment of an employee’s cause of injury, current
medical and rehabilitation treatments, current employment
situation, and identification of barriers that may affect an
injured employee’s capacity to undertake a rehabilitation
program.
Why refer?

To identify and coordinate rehabilitation and treatment
strategies to ensure that an employee can participate in a
rehabilitation program.

To establish good communication channels between the employee,
their treatment providers and the agent to ensure integration of
all injury management activities and a focus on return to work.
